Pediatric Home Service (PHS) is located in the Twin Lakes Corporate Center at 2800 Cleveland Avenue North, Roseville, MN 55113. Look for the PHS sign on the outside of the building.

Getting to PHS Map It!

1. Take I-35W to Roseville
   
2. Take EXIT #25A for County Road D
   
3. At the top of the exit (from either direction),
   turn right (east) onto County Road D
   
4. Turn right (south) onto Cleveland Avenue North
   
5. PHS is on the left side (east) of the road.
   About 0.7 miles south of County Road D
