We will need the specific class(es) and dates you would like to take.

In your email, we will need your name, phone number, class title(s) and date(s), and if possible, your employer's name. We will contact you to confirm your attendance.
Please note that:

• All participants must pre-register.
• Mandatory contact/teaching hours awarded for classes attended by healthcare professionals.
• Visa, MasterCard, or exact cash only. We cannot accept personal checks. Late arrivals may be denied admittance to class.
• All classes are located at 2800 Cleveland Avenue North, Roseville, MN 55113 unless otherwise noted.

Class Pricing Structure (effective January 2008)

• Free to our Families (non-healthcare professional caregiver of patient.)
• $5 per hour for equipment-based classes for healthcare professionals with PHS patients.
• $20 per hour for skills-based classes for healthcare professionals with PHS patients.
• $25 deposit required to hold CPR Recertification class request, which will be applied toward $75 class cost. 24-hour cancellation notice is needed to have deposit returned.
• $50 per hour for healthcare professionals with no PHS patient relationship.
• Special large group class arrangements on request.
