Pediatric Home Service has been accredited by The Joint Commission since 1994.

In-Home Asthma Education

Visits Include:

  • Environmental Assessment
  • Pulmonary Assessment
  • Educational Intervention

The custom education program includes information regarding:

  • Recognition of early warning signs and proper response
  • Medication usage and compliance
  • Inhaler technique or nebulizer therapy
  • Personal triggers
  • Peak flow monitoring
  • Asthma action plan

Responsibilities

The Asthma Educator helps the family and child better understand and manage their care.

Credentials

Service is provided by Registered Perinatal/Pediatric Respiratory Care Practitioners who are also Certified Asthma Educators.

In June 2003, reviewers from the Joint Commission evaluated PHS' In-Home Pediatric Asthma Program to assess its compliance with national standards and performance measurement expectations for the management of chronic care illnesses.

We voluntarily pursued this comprehensive, independent evaluation to enhance the safety and quality of care we provide.

The result of this evaluation was the achievement of the Gold Seal of Approval for Pediatric Asthma.

Geographical Coverage

Chronic Disease Service provides service for the 13 county metro area of Minneapolis/St. Paul, Minnesota and western Wisconsin.

Competency

The capability of each Asthma Educator is verified annually through extensive skills testing.

Continuing Education

Each member of Chronic Disease Service is required to augment their professional competence by accumulating 24 continuing education credits every 2 years, and obtaining recertification of their Certified Asthma Education credentials every 5 years.

Patient Education

Appropriate educational literature is given to each family depending on the service provided. We have an array of brochures for all of our products and many diagrams explaining the "how to's" of a particular piece of equipment or supply. Much of this literature is used in marketing to doctors, hospitals, clinics, and other referral sources.
