PHS recognizes the important and interconnected nature of pharmacy and infusion therapy services and prides itself on providing a collaborative effort for each individual child, working with pharmacists, infusion nurses, dietitians, respiratory staff, and most importantly, the family.

How does our multi-disciplinary team help coordinate care for the child?
By offering specialized infusion therapies integrated with pharmacy services, medical social services and nutrition management, as well as ostomy, wound, and diabetes care.
We’ve assembled a team of dedicated respiratory therapists, nurses, pharmacists, dietitians, social workers, educators, and an extensive support staff who work together to ensure that every child is safe, secure and certain to receive the same quality provided in the hospital.
These experienced caregivers provide nutritional support , education and medical social services, as well as enteral therapy, care planning, medical supply fulfillment, and the kind of customer service pediatric patients and their caregivers can count on.
Where do we provide care for the child?
In the child’s home, where they need us. Our multi-disciplinary team goes out to communities across the metro area and works with other homecare agencies around the state (service area map)
