Medication Therapy Management Program at PHS

We all know that caring for a child with medical complexities is a big job. Many patients in our population require multiple medications to manage chronic conditions. And oftentimes families are getting prescriptions from a variety of providers who are on the child’s care team. PHS has a service available to our MN Medicaid patients that can improve the outcomes of medication therapy.

Medication Therapy Management is a comprehensive assessment of all medications that the patient is taking, and the schedule they are taking them on.  We want to ensure the patient is getting the most out of their medication regimen, so this review includes prescriptions, non-prescription, and herbal medications. Our pharmacy team then groups all the information together, does a thorough review to look for drug interactions, and provides medication education on exactly what the medication is doing in the body.

The best part? You don’t need to leave your home! This service is all virtual. So long as you have an additional health care provider in your home and a computer with a camera or a smartphone with a camera, we will be able to conduct a virtual visit. Don’t have another health care provider in your home? No problem, we can schedule an in-office visit as well.

MEET THE MEDICATION THERAPY MANAGEMENT TEAM

  1. Our pharmacists have experience managing care for the complex pediatric population and understand that children process medications differently.
  2. Ensure your child’s medication schedule is optimized so they receive the most benefit from each of their prescribed medications.
  3. When a variety of medications are involved, safety comes top of mind. Medication Therapy Management minimizes the risk of drug interactions, keeping patients safe.

  1. Contact PHS (complete the form below or call and ask to talk to the pharmacy) to begin Medication Therapy Management.
  2. A pharmacist will schedule a virtual meeting with you.
  3. So long as you have another health care professional present in the home, the meeting can be conducted with your computer, tablet, or even cell phone. If not, we will schedule an in-office visit.

  1. A complete list of your child’s medications, including the dosages and schedule.
  2. An electronic device with video capabilities.

  1. Medical Assistance covers up to 8 medication therapy management meetings per year.
  2. Initial visits take up to 60 minutes, with follow up meetings taking 15-30 minutes.
  3. Commonly, patients use up to 3 review meetings per year depending on need (your exact schedule is determined following initial meeting).
  4. The state of MN requires another health care professional to be present in the home for any Medication Therapy Management visits that take place virtually, we can talk through these details on the initial call.
  5. Following the initial call, a summary and recommendation list will be built by the pharmacist. This document will be sent to both you and your providers for consistency of care and collaboration.
