Triad lubricant jelly recall

Triad, manufacturer of sterile lubricating jelly, recently contacted PHS to tell us they are concerned that some of their lubricating jelly product that recently shipped, may, in fact, not be sterile.

Please examine all the packets of Triad sterile lubricating jelly you have at home. There is a line of numbers and letters stamped in black on the front of the packet. Triad has indicated that packets stamped with a line that starts with 7, 8, 9 or 0 (like sampled picture above) should be discarded. If you are affected by this recall and must discard your supply of Triad sterile lubricating jelly purchased from PHS, please call PHS Customer Service (651-642-1825) to let us know the number of boxes or the approximate number of packets you have discarded and to discuss replacement options. There are 144 packets in a box so it’s possible that you will only be discarding a partial box.

If your Triad sterile lubricating jelly was purchased from PHS, we will replace these packets free of charge.  However, Triad has informed PHS that they are currently unable to supply PHS with replacement packets. PHS will send you the free replacement packets when Triad begins shipping the packets again. In the meantime, if you need this supply immediately, please contact PHS during regular business hours (Mon-Fri, 8a-5p) to discuss how you may obtain sterile lubricating jelly made by a different manufacturer.
