PHS collaborates with families to ensure a smooth transition, easing concerns and guiding them through each step. Use this roadmap to understand PHS services and navigate the journey with confidence.

PHS Admissions Process

A PHS intake specialist will answer your questions and gather the necessary information related to your child’s care. We collaborate with your providers to determine the required services and any necessary authorization requirements.

Home Assessment

Our primary goal is to keep patients safe at home. For example, if your child requires oxygen, we will visit your home before you switch home care providers or leave the hospital to address any potential safety issues.

Coordinated Care

PHS collaborates with internal and external care teams to provide education on supplies.

Section 3 (1)

Insurance and Billing

PHS billing specialists understand the complex, changing needs of pediatric patients. We are here to help guide you through understanding your coverage and help with payment plans.

We have contracts with all the major insurance providers in the area. That means for the majority of our patients, some or all of the service is covered. Work with your care coordinator to determine coverage.

Equipment Delivery and Setup

PHS clinicians work with your family to arrange equipment delivery and setup.

Caregiver Training

We ensure that all caregivers are fully competent, confident, and comfortable with your child’s care routine, equipment, and therapies.

Section 5 (1)

Ongoing Care

Information sharing is seamless. We provide access to the electronic chart when doctors and pharmacists need it. In the home, families and nurses can use a communication book, and families also have access to medical records through MyPHS Portal.

You can reach us day and night. Our experienced clinicians take your call and assess your child’s needs 24 hours a day, every day of the year
