Comprehensive Nursing Care – Keeping Kids at Home

The PHS nurses (trained RNs, LPNs and LVNs) – also called Private Duty Nursing (PDN) or Home Care Nursing (HCN) – provide one-on-one shift-based nursing services for children with technology-dependent health conditions in the comfort and convenience of their homes and communities. We make sure that each nurse is trained in the complexities of pediatric home care, as well as each child’s individualized health needs.

Our pediatric nursing services and specialties include:

  • Tracheostomy or ventilator care
  • Feeding tubes, catheter care, or ostomy care
  • Oxygen therapy
  • Medication administration
  • Complex intravenous therapy (medicine provided through an IV, central venous line, PICC line, or port in the home setting)
  •  School-based nursing care when appropriate
  •  Skilled Nursing Visits
    • Intermittent visits to provide education, perform lab draws, administer medication, dressing changes or other physician ordered cares.

All nursing care, skilled nursing services (aide-level care), treatment, and medications are delivered under physician orders. Services may vary by market and/or by payor authorization. Contact your local PHS location for a full list of available services.

  • Meet families to ensure a good fit before being placed
  • Are supported by an experienced clinical team 24/7
  • Empower families to become confident caregivers

Comprehensive Care Coordination

  • Coordinate services with care team to provide a seamless transition to services with PHS
  • Begins with regular communication and visits before care start
  • Regular communication with therapists, nurses, caregivers, and the physician’s care team

Transition to Home

We provide an individualized approach and collaborate with all parties involved to coordinate transition to home process before, during, and after discharge.
