Why PHS and How We Measure Success

With PHS, you get a coordinated pediatric team and clear proof it’s working.

  • One coordinated pediatric team—HCN, respiratory/DME, infusion/pharmacy, education, and social work—delivers smooth, proven care at home.
  • We track results: faster discharges, fewer avoidable hospital returns, higher caregiver readiness and satisfaction, and stronger provider feedback.
