Pediatric Home Respiratory Services, LLC d/b/a Pediatric Home Service (“PHS” “our” or “we”) cares deeply about our patients and takes seriously its obligation to safeguard information entrusted to us. On November 7, 2024, we discovered that an unauthorized third party had gained temporary access to our network. Upon learning of the situation, we immediately took steps to contain and remediate the incident and began an internal investigation. We also reported the incident to law enforcement and engaged a leading forensic security firm to assist in our investigation and confirm the security of our computer systems and network. The investigation determined that the unauthorized third party accessed certain files on our network between November 1, 2024 and November 8, 2024. After reviewing the files, we determined that, while the information varied by individual, the files contained individuals’ names, date of birth, contact information, Social Security number, medical information, and health insurance information.
In addition to the actions described above, we have taken steps to reduce the risk of this type of incident from occurring in the future, including enhancing our technical security measures. PHS has sent letters to potentially involved individuals for whom it has a valid mailing address and has arranged for complimentary credit monitoring and identify theft protection services for those individuals whose Social Security numbers were involved in the incident. To date we have no reason to believe that personal information potentially involved in this incident has been misused for the purpose of committing fraud or identity theft but as a precautionary measure individuals should remain vigilant to protect against other things, reviewing their account statements and monitoring credit reports closely. If individuals detect any suspicious activity they should notify the entity with which the account is maintained, and promptly report any fraudulent activity to proper law enforcement authorities, including the police and their state attorney general.
